RN Advocacy
With Premier Geriatric RN’s RN Advocacy, you have a Registered Nurse to provide oversight and coordinate care in the home, hospital, or facility. This service also includes health monitoring visits and coordination to facilitate continuity of care by acting as a liaison between health care professionals and services.
Client-centered care plans will be implemented and monitored for effectiveness. Communication will be ensured among all providers of care and family and assistance will be provided to help navigate through the healthcare system.
