The following Member Services are covered by the Annual Service Fee:
- Pediatric/Adolescent Care
-Full scope of Board-Certified Family Medicine care
-Newborn exams
-Acute care visits
-Preventative/counseling visits
-Well child exams (ate age intervals advised by the Centers for Disease Control and Prevention, American Academy of Pediatrics and the American Academy of Family Physicians
-Yearly age appropriate flu vaccine
- Adult Care
-Full scope of Board-Certified Family Medicine Care
-Yearly complete physical exam, based on guidelines of the U.S. Preventative Services Task Force (includes age-appropriate cancer/medical screenings based on Standard of Care guidelines
-Acute care visits
-Preventative/counseling visits
-Yearly flu vaccine
- 24/7 access to your Belleview Physician via phone, email, text
- Same-day or next day urgent care visits at Belleview’s office
- Minimum 30 minute/ one hour appointments (may be longer or shorter based on patient’s needs)
- Unlimited in-office visits during the term of the Agreement (Belleview’s office)
- Pre-travel consultations
- Acute breathing treatments (e.g. nebulizer treatments)
- Simple laceration repair
- Simple skin lesion removals (e.g. punch biopsy, skin tag removal, shave biopsy, elliptical incisions)
- Large joint injections (e.g. shoulder, knee, plantar fasciitis, epicondylitis, carpel tunnel, etc.)
- Rapid in-office diagnostic studies (e.g. Influenza A/B Swab, Group A Strep Swab, Urinalysis, Urine Pregnancy Tests, ECG)
- House calls and patient’s office visits may be incorporated at physicians discretion
- Cryotherapy (e.g. wart removal)
